VAPZ for medical professions
Are you a contracted healthcare provider? Then, under certain conditions, you are entitled to an annual allowance from RIZIV, which you can use for a guaranteed income contract and/or a social VAPZ (the free supplementary pension for the self-employed).

What is a VAPZ for medical professions, or the RIZIV contract?
Are you a contracted healthcare provider? Then, under certain conditions, you are entitled to an annual allowance from RIZIV. You can use this allowance for a guaranteed income contract and/or a social VAPZ (a so-called RIZIV contract). Practitioners of certain medical professions can, in addition to a VAPZ agreement, also take out a RIZIV agreement. This scheme exists for physicians, dentists, pharmacists, and physiotherapists who are fully or partially contracted. Since 2016, speech therapists and home nurses are also eligible for an allowance. The RIZIV agreement fully follows the tax treatment and the options of a social VAPZ. The only difference is that here it is not the (para)medical professional who makes the payment, but RIZIV.
Who is it for?
Physicians, dentists, pharmacists, physiotherapists, speech therapists, and home nurses. For certain professions, people with employee status can also claim a RIZIV allowance (e.g. physiotherapists and pharmacists). Branch 21 or Branch 23? Only Branch 21 is possible.
A VAPZ can only be taken out in Branch 21. What is the taxation of the payout?
On payout, a 3.55% RIZIV contribution and a 0 to 2% solidarity contribution are deducted from the capital. Beyond that, as with the VAPZ, the notional annuity system applies. As a result, for 10 to 13 years an amount of at most 5% of the total capital, reduced by a RIZIV contribution of 3.55%, is declared as 'notional' income.
